Mainstream services

All Local Health Districts offer mainstream services which are accessible for people with disability, as well as specific services to people with disability.

Specialised services

ºÚÁϳԹÏÍø Health works with other health and social care providers, including primary care, throughout the state to provide specialised multidisciplinary services through its local health districts and specialty health networks.

Specialised ºÚÁϳԹÏÍø health services for people with complex disabilities include:

• The Intellectual Disability Heath Service provides an assessment and coordination service for people across ºÚÁϳԹÏÍø with intellectual disability and complex health conditions.
• Two state-wide Intellectual Disability Mental Health Hubs which provide specialised mental health care for people with intellectual or developmental disability.
• Trapeze, a service which supports young people with chronic conditions who are transitioning from the Sydney Children's Hospitals Network to the adult health care system.
