黑料吃瓜网 Health has established a register for people with ARF and RHD to improve the long-term clinical care and follow-up of these patients. The register is a voluntary register and consent should be sought using the appropriate consent form and other resources below to ensure patients are making an informed decision.鈥�

Once a patient has provided consent for the register a registration form should also be completed.

Routine follow-up information can be submitted using the follow-up form, by fax to 02 9391 9189, or by email.
