Community Living Supports Service Model -June 2025

​â¶Ä‹â¶Ä‹â¶Ä‹â¶Ä‹S³Ü³¾³¾²¹°ù²â​â¶Ä‹â¶Ä‹â¶Ä‹

The service model for the Housing and Accommodation Support Initiative (HASI) and Community Living Supports (CLS) program has been reviewed and refreshed in preparation for the 2025-26 open grant opportunity. This was to ensure a contemporary model is in place to inform the upcoming open grant opportunity and future operations of the program. The refreshed service model will take effect from 1 July 2026.

Key outcomes from the service model review are that:

  • the HASI and CLS programs will be merged into a single program known as 'Community Living Supports' (CLS).
  • the service model has been refreshed and tailored to target consumers with severe and complex mental health needs.
  • the consumer journey remains evidence based and reflects a contemporary service that will build on the strengths and aspirations of eligible individuals.
​The CLS service model provides direction for community managed organisations and local health districts delivering the CLS program for people with severe mental health conditions under the auspices of the Mental Health Branch, ºÚÁϳԹÏÍø Ministry of Health.

CLS is a holistic and culturally response service that builds on the strengths and aspirations of people with severe mental health conditions, working together to create meaningful goals and personal change for a fulfilling life within their communities.
