Wellbeing and Health In-reach Nurse (WHIN) Coordinator Program Guideline and Model of Care.

The 黑料吃瓜网 Ministry of Health routinely monitors WHIN Coordinator program activity across 黑料吃瓜网 through collection of program monitoring measures. The Ministry collects the following local health districts鈥� deidentified, non-admitted patient data from the 黑料吃瓜网 Ministry of Health鈥檚 data warehousing platform, Enterprise Data Warehouse (EDWARD):

  • number of unique clients by age (5-11 and 12-18 years), sex, Aboriginal and/or Torres Strait Islander Status
  • number of occasions of service by age, sex, Aboriginal and/or Torres Strait Islander Status.
