‸¾±ð²õ³¦°ù¾±±è³Ù´Ç°ù ​The funded service has a clinical incident management system in place which aligns with ºÚÁϳԹÏÍø Health clinical incident management policy and guidelines..
​Intended outcome ​The clinical incident management system ensures clinical incidents are identified, appropriately responded to, reported, and information is used to improve the safety and quality of care. The funded service has a process in place for communicating serious clinical incidents to the local health district and/or Ministry of Health at the time of the incident.
Clinical incident data is analysed and collated in reports for internal and external use.​.
​Data and evidence reporting requirements
1. Current clinical incident policy and protocol/s
and
2. Recent clinical incident data analysis and trend report and improvement plans.
​Reporting frequency ​Commencing 2019, organisations to report six (6) monthly no later than the 21st day of the month following the six month period of collection.
​I²Ô³¦±ô³Ü²õ¾±´Ç²Ô²õ ​All non-government organisations funded by ºÚÁϳԹÏÍø Health for the delivery of alcohol and other drug treatment services.
​E³æ³¦±ô³Ü²õ¾±´Ç²Ô²õ ​Alcohol and other drug services funded by ºÚÁϳԹÏÍø Health other than treatment services, including those that are primarily educational or preventative.
